The impacts of habitat, microclimate, and experimental confinement on survival rates of unfed adult blacklegged ticks, Ixodes scapularis Say, were studied in field enclosures in southcentral and northwestern Connecticut At both locations, 2 enclosures were placed in each of 3 different habitats (field, forest canopy, and forest/field edge). Forty wild-caught adult ticks (20 males, 20 females) were placed in each enclosure. At one site, another 40 ticks were confined to nylon mesh bags placed inside each enclosure. Soil temperature, ground-level air temperature and relative humidity were measured within each habitat. The number of ticks surviving within each enclosure was monitored 1 or 2 times per week. Ticks that were confined in nylon bags had a lower survival rate than ticks that were able to move freely within the enclosures. Ticks survived longer in edge and forest habitats than in open fields, which were characterized by greater extremes in air temperature, soil temperature, relative humidity, and vapor pressure deficit than the other 2 habitats. The mean daily survival rates of free-ranging I. scapularis were negatively related to air temperature, vapor pressure deficit, and the coefficient of variation of relative humidity.' This article gives an overview of the current state of knowledge of risk factors and provides commentary on the role of health promotion in the prevention of a climate change-related increase in elderly heat casualties. Methods: A search of peer-reviewed medical and epidemiological literature and community health websites was conducted in order to gain an in-depth understanding of heat-susceptibility in the elderly and preventive strategies. Key search words included: elderly, aged, older, heat, thermoregulation, heat wave, mortality, heat effects, dehydration, heat-related illness, adaptation, adaptive capacity. Results: The reasons underlying reduced heat tolerance in this group are multi-faceted, comprising physiological, social and behavioural limitations, with comorbidities and polypharmacy being contributing factors. Additionally, some older persons may be unable or reluctant to undertake adaptations necessary to maintain thermal homeostasis due to diminished awareness of the heat, lowered thirst sensation, mobility or cognitive impairments, a lowered perception of risk, or economic concerns. Conclusion: With older persons in poor health being particularly vulnerable to heat, preventive messages need to promote protective behaviours and help build resilience as temperatures rise. METHODS: County-level daily emergency department visit counts and daily mean temperatures for the period 1/1/2007-12/31/2008 were merged to form a time-series data structure. Incidence rates were calculated by sex, age group, region, day of week, and month. Incidence rate ratios were estimated using categorical and linear spline Poisson regression models and heterogeneity of the temperature-emergency department visit association was assessed using product interaction terms in the Poisson models. RESULTS: In 2007-2008, there were 2539 emergency department visits with heat-related illness as the primary diagnosis. Incidence rates were highest among young adult males (19-44 year age group), in rural counties, and in the Sandhills region. Incidence rates increased exponentially with temperatures over 15.6 degrees C (60 degrees F). The overall incidence rate ratio for each 1 degrees C increase over 15.6 degrees C in daily mean temperature was 1.43 (95%CI: 1.41, 1.45); temperature effects were greater for males than females, for 45-64 year olds, and for residents of rural counties than residents of urban counties. CONCLUSIONS: As heat response plans are developed, they should incorporate findings on climate effects for both mortality and morbidity. While forecast-triggered heat health warning systems are essential to mitigate the effects of extreme heat events, public health preparedness plans should not ignore the effects of more persistently observed high environmental temperatures like those that occur throughout the warm season in North Carolina." The randomized trial is one study design used to evaluate risks attributable to drinking water. In this paper, we review all published randomized trials of drinking water interventions in industrialized countries conducted among general immunocompetent populations. We then present an approach to estimating the incidence (number of cases) of AGI attributable annually to drinking water. To develop a national estimate, we integrate trial results with the estimated incidence of AGI using necessary assumptions about the estimated number of residents consuming different sources of drinking water and the relative quality of the water sources under different scenarios. Using this approach we estimate there to be 4.26-11.69 million cases of AGI annually attributable to public drinking water systems in the United States. In New Jersey, thousands of families were made homeless and entire communities were destroyed in the worst disaster in the history of the state. The economic impact of Sandy was huge, comparable to Hurricane Katrina. The areas that sustained the most damage were the small- to medium-sized beach communities along New Jersey's Atlantic coastline. Six months following the hurricane, we conducted a random telephone survey of 200 adults residing in 18 beach communities located in Monmouth County. We found that 14.5% (95% CI = 9.9-20.2) of these residents screened positive for PTSD and 6.0% (95% CI = 3.1-10.2) met criteria for major depression. Altogether 13.5% (95% CI = 9.1-19.0) received mental health counseling and 20.5% (95% CI = 15.1-26.8) sought some type of mental health support in person or online, rates similar to those reported in New York after the World Trade Center disaster In multivariate analyses, the best predictors of mental health status and service use were having high hurricane exposure levels, having physical health limitations, and having environmental health concerns. Research is needed to assess the mental health status and service use of Jersey Shore residents over time, to evaluate environmental health concerns, and to better understand the storm's impact among those with physical health limitations."
